In this dual clinical sales and admissions leadership role, you will leverage hospital relationships to identify prospective sub-acute and long-term care patients, acting as a primary advisor to hospital staff and physicians to facilitate smooth transfers.

• Minimum 2+ years in a SNF/LTC setting in a liaison, admissions, or healthcare business development role.
• Established hospital relationships with case management teams in Brooklyn and Queens.
• Bachelor’s degree in healthcare administration, marketing, nursing, or related field preferred; active clinical credential is a strong plus.
• Industry knowledge of acute hospital discharge workflows, SNF/LTC admission criteria, Medicare Part A, Managed Medicaid, and commercial insurance pre-authorizations.

• Territory & network expansion: Maintain relationships with hospital teams across Brooklyn and Queens.
• Clinical pre-screening: Perform pre-admission evaluations and chart reviews to assess medical stability and needs.
• Census & referral generation: Convert clinical leads into admissions to meet census targets.
• Patient & family guidance: Explain sub-acute rehab programs and answer clinical questions in hospital settings.
• Interdisciplinary collaboration: Coordinate with admissions, nursing leadership, and therapy teams for hand-offs and pre-authorizations.
• Market intelligence: Track discharge patterns and present educational sessions to hospital case management teams.
