This dual clinical sales and admissions leadership role leverages hospital relationships to identify prospective SNF and LTC patients and guide transitions. You will work with case managers, discharge planners, social workers, and physicians, perform pre-admission evaluations, secure pre-authorizations, and collaborate with admissions teams to expand the

Qualifications

• Minimum 2+ years of direct SNF/LTC experience in a liaison or admissions role.
• Proven hospital relationships with case management teams in Brooklyn and Queens.
• Bachelor’s degree preferred; active clinical credential is a plus.
• Deep understanding of discharge workflows and SNF/LTC criteria.
• Strong consultative sales, clinical evaluation, communication and accountability.

Responsibilities

• Territory & network expansion across Brooklyn and Queens hospital systems.
• Clinical pre-screening and chart reviews for medical stability and fit.
• Convert leads to admissions to meet census targets.
• Guide patients and families on sub-acute programs and transitions.
• Collaborate with admissions, nursing and therapy leaders for hand-offs.
• Monitor local discharge trends and present to hospital case management.
