Job summary

Zaddy Solutions is seeking an experienced Hospital Liaison to direct post-acute intake and drive census growth across major NYC health systems. This dual clinical sales and admissions leadership role emphasizes pre-admission evaluations, hospital-to-facility transitions, and expanding the facility’s referral footprint.

Qualifications

• Minimum of 2+ years direct experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting as a liaison, admissions, or healthcare business development professional.
• Established hospital relationships with case management teams in Brooklyn and Queens markets.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field; clinical credential is a strong plus.
• Knowledge of Medicare Part A, Medicaid, and commercial insurance pre-authorizations.

Responsibilities

• Territory expansion by cultivating relationships with hospital staff across Brooklyn and Queens.
• Perform pre-admission evaluations and chart reviews to assess medical stability and care needs.
• Generate census and referrals to meet monthly targets and occupancy goals.
• Guide patients and families in hospital settings about sub-acute programs and transitions.
• Collaborate with admissions teams, nursing directors, and therapy leaders on care plans and pre-authorizations.
• Monitor market trends and deliver educational presentations to hospital management teams.
