Job summary

United Future in New York is seeking an experienced Hospital Liaison to direct post-acute intake and drive census growth across NYC health systems.

In this dual clinical sales and admissions leadership role, you will leverage hospital relationships to identify prospective SNF and LTC patients, perform pre-admission evaluations, streamline transfers, and expand the facility's referral footprint.

Qualifications

• Minimum 2+ years in SNF/LTC liaison, admissions, or healthcare business development.
• Active hospital relationships with case management teams in Brooklyn and Queens.
• Knowledge of Medicare Part A, Managed Medicaid, and pre-authorizations.

Responsibilities

• Territory & network expansion across Brooklyn and Queens hospital systems.
• Perform pre-admission evaluations and chart reviews for medical fit.
• Convert clinical leads into admissions to meet census targets.
• Guide patients and families on sub-acute rehabilitation options.
• Collaborate with admissions, nursing, and therapy for hand-offs.
• Monitor local discharge trends and educate hospital teams.
