Job summary

Talent With Heart in New York is seeking an experienced Hospital Liaison to direct post-acute intake and drive census growth across major NYC health systems. In a dual clinical sales and admissions leadership role, you will leverage hospital relationships to identify patients and streamline transitions.

You will pre-screen, coordinate pre-authorizations, and collaborate with nursing and therapy teams to expand referrals while meeting monthly census targets.

Qualifications

• Minimum 2+ years in SNF/LTC in a liaison/admissions role.
• Active hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred; clinical credential is a strong plus.
• Knowledge of discharge workflows, SNF/LTC admission criteria, Medicare Part A, Medicaid, and pre-authorizations.
• Strong consultative sales ability, clinical evaluation, communication, and accountability.

Responsibilities

• Territory & Network Expansion: Build and nurture high-value relationships with hospital case managers, discharge planners, social workers, and hospitalists across health systems in Brooklyn and Queens.
• Clinical Pre-Screening: Conduct pre-admission evaluations and chart reviews to assess stability and care needs.
• Census & Referral Generation: Convert clinical leads into admissions to meet monthly targets.
• Patient & Family Guidance: Advise patients and families on sub-acute programs and transitions in hospitals.
• Interdisciplinary Collaboration: Coordinate with admissions teams, nursing directors, and therapy leaders for hand-offs and pre-authorizations.
• Market Intelligence: Monitor local trends and present education to hospital teams.
