Job summary

Ashford Connex, LLC is seeking a Hospital Liaison to direct post-acute intake and drive census growth across major NYC health systems. This dual clinical sales and admissions leadership role leverages hospital relationships to identify prospective SNF and LTC patients and expand the facility's referral footprint.

You will perform pre-admission evaluations, streamline patient transfers, and work with hospital case managers, discharge planners, social workers, and physicians to ensure smooth

Qualifications

• Minimum 2+ years of direct SNF/LTC experience in a liaison/admissions/business development role.
• Established hospital relationships with Brooklyn and Queens case management teams.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred; active clinical credential is a strong plus.
• Deep understanding of acute hospital discharge workflows, SNF/LTC admission criteria, Medicare Part A, Managed Medicaid, and commercial insurance pre-authorizations.
• Strong consultative sales ability, decisive clinical evaluation skills, excellent communication and professional accountability.

Responsibilities

• Territory & network expansion with hospital teams across Brooklyn and Queens.
• Clinical pre-screening and chart reviews for patient eligibility and care needs.
• Convert leads into admissions to meet census and occupancy targets.
• Guide patients and families in hospital settings about sub-acute programs and transitions.
• Coordinate with admissions, nursing, and therapy leaders to communicate care plans and secure pre-authorizations.
• Monitor local healthcare trends and educate hospital case management teams.
