This dual clinical sales and admissions leadership role leverages hospital relationships to identify prospective SNF/LTC patients and acts as a primary advisor to discharge planners, social workers, and physicians. Qualifications

• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred.
• Active clinical credential (RN, LPN, MSW, LCSW, PT, or OT) is a strong plus.
• Minimum 2+ years SNF/LTC experience in liaison, admissions, or healthcare business development.
• Proven hospital relationships with case management teams in Brooklyn and Queens.

Responsabilités

• Maintain high-value relationships with hospital case managers, discharge planners, social workers, and hospitalists across health systems in Brooklyn and Queens.
• Perform thorough pre-admission evaluations and chart reviews to determine medical stability and fit for SNF or sub-acute care.
• Convert clinical leads into admissions to meet census and occupancy targets.
• Consult with patients and families in hospital to outline sub-acute rehab programs and transitions.
• Collaborate with admissions teams, nursing directors, and therapy leaders to coordinate care plans and pre-authorizations.
• Monitor local discharge trends and deliver educational presentations to hospital management teams.
