Job summary

Key People is seeking a Hospital Liaison in New York City to lead post-acute intake and drive census growth across major hospital networks. The role blends clinical oversight with admissions leadership, leveraging hospital relationships to identify sub-acute and long-term care opportunities and to streamline patient transfers.

Qualifications

• Minimum 2+ years SNF/LTC experience in liaison/admissions role.
• Active hospital relationships with Brooklyn/Queens case management teams.
• Bachelor's degree in Healthcare Admin, Marketing, Nursing, or related field; clinical credential (RN/LPN/MSW/LCSW/PT/OT) is a plus.
• Deep knowledge of hospital discharge workflows and SNF/LTC criteria, Medicare Part A, Managed Medicaid, and pre-authorizations.
• Strong consultative sales, clinical evaluation, communication, and accountability.

Responsibilities

• Territory & Network Expansion: Build high-value relationships with hospital teams in Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews for medical fit.
• Census & Referral Generation: Convert leads to admissions to meet census targets.
• Patient & Family Guidance: Explain sub-acute programs and coordinate smooth transitions.
• Interdisciplinary Collaboration: Align with admissions, nursing, and therapy leaders for care plans.
• Market Intelligence: Track discharge trends and present to hospital management.
