Workforce Recruiter in New York City seeks an experienced Hospital Liaison to direct post-acute intake and drive census growth across major NYC health systems, including Brooklyn and Queens. This dual clinical sales and admissions leadership role leverages hospital relationships with case managers, discharge planners, social workers, and physicians to optimize referrals and transitions.

You will perform pre-admission evaluations, streamline patient transfers, and expand the facility's referral

Qualifications

• Minimum 2+ years in SNF/LTC liaison, admissions or healthcare business development.
• Active hospital relationships in Brooklyn and Queens markets.
• Bachelor’s degree in a related field; clinical credential a strong plus.
• Deep knowledge of hospital discharge workflows and pre-authorizations.
• Strong consultative sales ability and accountable professional conduct.

Responsabilités

• Territory & Network Expansion: cultivate hospital relationships in Brooklyn and Queens.
• Clinical Pre-Screening: evaluate medical stability and care needs for admissions.
• Census & Referral Generation: convert leads into admissions to meet targets.
• Patient & Family Guidance: discuss sub-acute programs and transitions in hospital.
• Interdisciplinary Collaboration: coordinate with admissions, nursing and therapy.
• Market Intelligence: monitor discharge patterns and educate hospital teams.
