Job summary

Saint Cyr Partners seeks a Hospital Liaison to lead post-acute intake and drive census growth across NYC health systems. This dual clinical sales and admissions leadership role uses hospital relationships to identify referrals and coordinate patient transfers.

Qualifications

• Minimum 2+ years in SNF/LTC liaison or admissions roles.
• Established hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in healthcare field; clinical credential a plus.
• Deep understanding of discharge workflows and pre-authorizations.

Responsibilities

• Maintain high-value hospital relationships across NYC health systems.
• Perform pre-admission evaluations and chart reviews for suitability.
• Convert clinical leads into admissions to meet census targets.
• Guide patients and families through sub-acute program options.
• Collaborate with admissions, nursing, and therapy teams on care plans.
• Monitor discharge patterns and educate hospital case management.
