Job summary

LRSolutions, LLC is seeking a Hospital Liaison to drive post-acute intake and census growth across major NYC health systems. This dual clinical sales and admissions leadership role involves pre-admission evaluations, coordinating transfers, and expanding the facility’s referral footprint.

You will work with hospital case managers, discharge planners, and physicians to ensure smooth transitions and robust referral generation, focusing on Brooklyn and Queens.

Qualifications

• Minimum of 2+ years direct SNF/LTC experience in a liaison/admissions role.
• Active hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field.
• Active clinical credential (RN, LPN, MSW, LCSW, PT, or OT) is a strong plus.
• Strong consultative sales ability and excellent communication.

Responsibilities

• Territory & Network Expansion: Maintain relationships with hospital teams in Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews.
• Census & Referral Generation: Convert leads into admissions to meet targets.
• Patient & Family Guidance: Explain sub-acute programs and transitions to families.
• Interdisciplinary Collaboration: Coordinate with admissions, nursing, and therapy.
• Market Intelligence: Track discharge patterns and present to hospital teams.
