Job summary

Hire Consulting Services seeks a Hospital Liaison to lead post-acute intake and drive census growth across NYC health systems. This full-time on-site role focuses on clinical sales and admissions leadership, utilizing hospital relationships to expand referrals and streamline patient transfers.

You will work with case managers, discharge planners, social workers, and physicians to pre-screen patients, coordinate transitions, and support pre-authorizations, ensuring smooth hand-offs across teams.

Qualifications

• 2+ years SNF/LTC liaison or admissions experience.
• Active hospital relationships with case management in Brooklyn/Queens.
• Bachelor's degree in Healthcare Administration, Marketing, Nursing, or related field; clinical credentials (RN/LPN/MSW/LCSW/PT/OT) are a strong plus.

Responsibilities

• Territory & Network Expansion: Maintain high-value relationships with hospital teams in Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews for medical stability and care needs.
• Census & Referral Generation: Convert leads into admissions to meet census and occupancy targets.
• Patient & Family Guidance: Advise patients and families on sub-acute programs and transitions.
• Interdisciplinary Collaboration: Coordinate with admissions, nursing, and therapy to hand off care plans and secure pre-authorizations.
• Market Intelligence: Track discharge patterns and present educational insights to hospital management.
