RON SUNSHINE ASSOCIATES LLC is seeking a Hospital Liaison to direct post-acute intake and drive census growth across NYC health systems. This role blends clinical insight with sales leadership, engaging hospital case managers, discharge planners, social workers, and physicians to optimize referrals.

You will perform pre-admission evaluations, streamline patient transfers, and expand the facility’s referral footprint while collaborating with admissions teams and therapy leaders to ensure smooth

Qualifications

• Minimum 2+ years direct SNF/LTC liaison or business development experience.
• Proven hospital relationships with case management teams in NYC markets.
• Education credentials and clinical background are a strong plus.

Responsabilités

• Develop and expand hospital relationships across Brooklyn and Queens to drive census growth.
• Perform pre-admission evaluations, coordinate transfers, and secure pre-authorizations.
• Coordinate with admissions teams and therapy leaders to ensure seamless hand-offs.
• Educate hospital staff on sub-acute programs and transition workflows.
• Monitor discharge patterns and present market insights to leadership.
