Modern Machining Solutions Inc. is seeking a Hospital Liaison in New York to lead post-acute intake and drive census growth across major NYC health systems.

This dual clinical sales and admissions leadership role focuses on hospital relationships, pre-admission evaluations, and seamless patient transitions. You will expand the referral footprint by collaborating with case managers, discharge planners, and physicians, while coordinating with nursing and therapy leaders to ensure smooth hand-offs

Qualifikationen

• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred.
• Active clinical credential (RN/LPN/MSW/LCSW/PT/OT) is a strong plus, though non-clinical candidates with SNF referral production will be fully considered.

Aufgaben

• Territory & Network Expansion: Maintain relationships with hospital case managers and discharge planners across Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews to assess medical stability and care needs.
• Census & Referral Generation: Convert leads into admissions to meet census and occupancy targets.
• Patient & Family Guidance: Explain sub-acute rehab programs to patients and families in hospital settings.
• Interdisciplinary Collaboration: Work with admissions, nursing, and therapy leaders to coordinate care plans and pre-authorizations.
• Market Intelligence: Monitor discharge trends and present to hospital management teams.
