Top Echelon Software LLC is seeking a Hospital Liaison to direct post-acute intake and grow census across NYC health systems. The role blends clinical sales leadership with admissions leadership, targeting Brooklyn and Queens hospitals.

You will work with case managers, discharge planners, social workers, and physicians to evaluate patients, streamline transfers, and expand referral footprints, while coordinating with nursing and therapy leadership.

Qualificações

• Minimum 2+ years in SNF/LTC in a liaison/admissions role.
• Active hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Admin, Marketing, Nursing or related field; RN/LPN/MSW/LCSW/PT/OT a plus.
• Knowledge of Medicare Part A, Medicaid, and pre-authorizations.
• Strong consultative sales, clinical evaluation, and communication skills.

Responsabilidades

• Expand high-value hospital networks across Brooklyn and Queens.
• Perform pre-admission evaluations and chart reviews.
• Drive census and referrals to exceed targets.
• Guide patients and families through sub-acute programs.
• Collaborate with admissions, nursing, and therapy teams.
