Job summary

H.T. Prof Executive Search in New York seeks an experienced Hospital Liaison to direct post-acute intake and drive census growth across Brooklyn and Queens health systems.

In this dual clinical sales and admissions leadership role, you will build hospital relationships, perform pre-admission evaluations, streamline transfers, and expand the facility's referral footprint. Base compensation ranges from $90,000 to $130,000 per year, with full-time benefits and mileage reimbursement.

Qualifications

• Minimum 2+ years SNF/LTC liaison/admissions experience.
• Active hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Admin/Marketing/Nursing or related field.
• Active clinical credential is a strong plus; non-clinical candidates welcome.
• Knowledge of Medicare Part A, Medicaid, and pre-authorizations.

Responsibilities

• Territory & Network Expansion with hospital teams in Brooklyn/Queens.
• Clinical Pre-Screening and chart reviews to assess medical stability.
• Census & Referral Generation to exceed monthly targets.
• Patient & Family Guidance in hospital to outline rehab programs and transitions.
• Interdisciplinary Collaboration with admissions, nursing, and therapy leaders.
• Market Intelligence: monitor trends and educate hospital teams.
