This dual clinical sales and admissions leadership role leverages hospital relationships to pre-screen patients, streamline transfers, and expand the facility's referral footprint, with a base salary of $90,000–$130,000 per year.

Qualifications

• Bachelor’s degree and relevant healthcare experience required.
• SNF/LTC liaison or admissions experience preferred.
• Proven hospital relationship network in Brooklyn and Queens.

Responsibilities

• Develop and expand hospital relationships with case managers and discharge planners.
• Perform pre-admission evaluations and chart reviews for admissions.
• Drive census growth and manage referrals across NYC health systems.
• Collaborate with nursing, admissions, and therapy leadership on care plans.
• Monitor discharge patterns and present market insights to leadership.
