The Keystone Distinction Group is seeking a Hospital Liaison to direct post-acute intake and drive census growth across Brooklyn and Queens in a dual clinical sales and admissions leadership role.

You will leverage hospital relationships to identify prospective patients, perform pre-admission evaluations, and coordinate transfers with hospital case managers and physicians to ensure smooth hand-offs. This is a full-time on-site role with competitive compensation and benefits.

Formación

• Bachelor's degree preferred; active clinical credential is a strong plus.
• Minimum 2+ years SNF/LTC experience in a liaison, admissions, or healthcare business development role.
• Strong consultative sales ability, decisive clinical evaluation skills, and excellent communication.

Responsabilidades

• Territory & Network Expansion: Maintain and cultivate hospital relationships across health systems in Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews to determine medical stability and fit.
• Census & Referral Generation: Convert clinical leads into admissions to hit census and occupancy targets.
• Patient & Family Guidance: Advise patients and families on sub-acute programs and transitions.
• Interdisciplinary Collaboration: Coordinate with admissions teams, nursing, and therapy leaders for care plans and pre-authorizations.
• Market Intelligence: Monitor local trends and present to hospital management.
