Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• CPR certification is required.
• Minimum 1 year of professional nursing experience, preferably in hospice/home health/public health.
• Must be willing to drive and have reliable transportation.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing the nursing process.
• Develops and revises the plan of care with patient and family based on changes in status.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family and provides health instructions as appropriate.
• Acts as Case Manager to coordinate patient care for assigned caseload.
• Participates in on-call duties and ensures arrangements for equipment and services.
• Supervises ancillary personnel and commits to continuing education.
