Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, direct and coordinate hospice care in patients' homes and communities. The RN will perform assessments, develop plans of care, administer medications, and educate patients and families.

Qualified candidates hold an RN license and NLN-accredited education; experience in home health/hospice is preferred. The role includes coordinating care with an interdisciplinary team and may involve on-call duties.

Qualifications

• RN license required in state of practice.
• Graduate from NLN-accredited nursing program.
• Experience in home health, hospice, or public health preferred.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments.
• Develop and revise Plan of Care with nursing diagnoses and goals.
• Administer medications and treatments per physician orders.
• Educate patient and family on care, self-care techniques, and safety.
• Coordinate patient care as Case Manager with interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are available.
