Job summary

Affinity Hospice Management LLC is seeking an experienced Registered Nurse to plan, organize and direct hospice care in home settings and communities. The role emphasizes community health education and builds on nursing resources to meet patient and family needs.

The RN will complete comprehensive patient assessments, develop and revise care plans, administer medications, educate families, and coordinate care with an interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• Active Registered Nurse license in Florida.
• Minimum 1 year of professional nursing experience within the last 3 years; or a BSN with 2 years’ experience in public health, home care, or hospice (preferred).
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Delivers professional nursing care using the nursing process.
• Develops and revises plans of care with patient and family involvement.
• Administers medications and treatments per physician orders.
• Provides patient and family education and counseling on nursing and self-care.
• Acts as case manager to coordinate care for assigned caseload.
• Maintains accurate clinical notes and documents changes in patient condition.
• Maintains safe environment and coordinates interdisciplinary team communication.
