Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, direct, and coordinate hospice care in home and community settings. The role emphasizes nursing expertise, patient-centered care, and involvement with families in care planning and education.

The ideal candidate will have an active RN license, CPR certification, and 1+ year of recent nursing experience, preferably in home health or hospice.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in good standing.
• Minimum 1 year of professional nursing experience within the last 3 years, or a BSN with 2 years in public health/home care/hospice.
• Must be a licensed driver with a reliable vehicle.
• CPR certification required (hands-on verification).
• Excellent observation, verbal and written communication skills; problem solving ability.
• Ability to lift and transfer patients; ability to stand for long periods.

Responsibilities

• Complete an initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnoses.
• Implement preventive and rehabilitative nursing procedures; administer prescribed medications.
• Counsel patient and family to meet nursing needs.
• Provide health care instructions per assessment and plan.
• Assist with activities of daily living and promote patient independence.
• Act as Case Manager for assigned caseload.
• Maintain accurate clinical notes; communicate with physician; coordinate with interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise ancillary personnel and pursue ongoing professional development.
