Affinity Hospice Management LLC is seeking an experienced Registered Nurse to plan, coordinate, and direct hospice care in patients' homes and communities. You will perform comprehensive assessments, develop and revise care plans, administer medications, and educate patients and families in end-of-life care.

The ideal candidate has RN licensure in good standing, 1+ year of applicable nursing experience, and CPR certification.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN with license in good standing to practice.
• Experience: minimum 1 year as a professional nurse in last 3 years, or BSN with 2 years’ experience including public health/home care/hospice preferred.
• No management experience required; may supervise hospice aides.
• Offer contingent on references and background checks.
• Excellent observation, verbal and written communication, problem solving, and nursing skills per competency checklist.
• Must be a licensed driver with insured vehicle.
• CPR certification required (hands-on, not purely online).

Responsabilités

• Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status; write and initiate plan of care; revise as needed.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnoses and goals.
• Develop a care plan including patient and family in planning.
• Administer medications and treatments per physician plan of care.
• Counsel patient and family on nursing needs and self-care techniques.
• Provide health instructions and support activities of daily living.
• Act as Case Manager as assigned; coordinate patient care for caseload.
• Participate in on-call duties; arrange equipment and services.
• Supervise ancillary personnel and delegate tasks.
• Pursue ongoing professional development and continuing education.
• Fulfill case assignments and participate in quality/performance improvement teams.
