Job summary

Qualifications

• Graduate of NLN-accredited school of nursing.
• Current RN licensure in state.
• 1–2 years hospice/home health experience preferred.
• Management experience not required.

Responsibilities

• Assess patients and families to determine hospice needs.
• Develop and implement care plans in collaboration with the team.
• Provide nursing care and administer prescribed treatments.
• Educate patients and families on self-care and disease management.
• Coordinate with interdisciplinary group to ensure continuity of care.
• Participate in on-call duties as required.
