Job summary

Affinity Hospice Management LLC seeks a committed registered nurse to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education and collaboration with the interdisciplinary team.

The RN will perform comprehensive assessments, develop and adjust care plans, administer medications, counsel patients and families, and participate in ongoing quality improvement activities. A valid RN license and CPR are required.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse with license in good standing.
• One year of professional nursing experience within the last three years preferred, public health/home care/hospice exposure.
• Ability to meet driving requirements and have CPR certification.

Responsibilities

• Assess patient and family to determine hospice needs and develop a care plan.
• Provide professional nursing care and administer physician-ordered treatments.
• Document patient status and care in clinical notes; communicate with physicians and care team.
• Coordinate patient care as Case Manager when assigned; participate in on-call duties.
