Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home settings. The role emphasizes community health education and coordinating care for individuals and families within their homes and communities.

The RN will perform comprehensive assessments, develop and implement care plans, administer medications, and educate patients and families on self-care and disease management. Collaboration with the interdisciplinary team is essential.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN license + at least 1 year of professional nursing experience in last 3 years; 2 years preferred in public health/home care/hospice.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs and plan of care.
• Provide professional nursing care using the nursing process; administer medications and treatments as prescribed.
• Develop and revise care plans with patient and family involvement; reassess regularly.
• Coordinate care as case manager when assigned; ensure continuity and safety of care.
• Communicate with physicians and health team; document clinical notes and changes.
• Participate in on-call duties and QA/performance improvement activities.
