Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize, and direct hospice care in home and community settings. The role emphasizes community health education, nursing assessment, care planning, and collaboration with families and the interdisciplinary team.

Responsibilities include comprehensive patient assessments, developing and updating plans of care, administering prescribed treatments, and coordinating with physicians and caregivers to ensure quality hospice care

Qualifications

• Graduate of an accredited school of nursing and licensed as an RN.
• Must possess CPR certification and maintain clinical competency.
• Excellent observation, verbal and written communication skills.
• Ability to lift, position patients and drive as required for home hospice.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments.
• Develop and revise the plan of care with input from patient and family.
• Administer medications and treatments per physician orders.
• Coordinate care as Case Manager within the hospice team.
• Provide patient/family education and support in home settings.
• Participate in on-call duties and ensure equipment is available.
