Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. You will apply nursing processes, assess patients, and collaborate with families to meet end-of-life care needs.

The role emphasizes community health education, care plan development, and coordination with an interdisciplinary team, including on-call responsibilities and driving to patient homes.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing to practice.
• Minimum 1 year of professional nursing experience in the last 3 years (or equivalent).
• Experience in public health/home care/hospice preferred.
• Excellent observation, verbal and written communication skills.
• Ability to supervise hospice aides when required.
• Must be able to lift/move patients and supplies.
• Licensed driver with insured vehicle.
• CPR certification required with hands-on demonstration.

Responsibilities

• Performs comprehensive initial and ongoing patient assessments to determine hospice needs.
• Provides nursing care using the nursing process.
• Develops and revises plan of care with patient/family involvement.
• Initiates and revises the care plan as patient needs change.
• Writes and uses health data to establish nursing diagnoses.
• Coordinates care as Case Manager when assigned.
• Delivers patient/family education and self-care guidance.
• Maintains safe environment and continuity of care.
• Participates in on-call duties and team meetings.
