Job summary

Affinity Hospice is seeking a Registered Nurse Case Manager to plan, direct and provide high-quality hospice care in home settings. This role emphasizes community health education and coordination with families to deliver individualized care.

The RN will supervise the interdisciplinary team, ensure adherence to care plans, and maintain meticulous documentation while promoting the agency's philosophy and patient-centered goals.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect both staff and patient.
