Job summary

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family.
• Plan, implement and revise plan of care with physician directions.
• Administer medications and treatments per physician’s orders.
• Counsel patient and family; provide health instructions.
• Maintain accurate clinical notes and coordinate with physicians and caregivers.
• Participate in on-call duties and quality improvement activities.
