Qualifications

• RN license in good standing is required.
• Minimum 1 year of professional nurse experience within the last 3 years; public health/home care/hospice exposure preferred.
• Must have valid driver's license and CPR certification.

Responsibilities

• Complete initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family involvement.
• Administer medications and treatments as prescribed.
• Coordinate care as Case Manager when assigned by Clinical Supervisor.
• Document clinical notes and communicate with physicians and care team.
• Attend interdisciplinary group meetings and participate in quality improvement.
