Job summary

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Registered nurses shall have a minimum of one year of experience as a professional nurse within the last three years; OR have a baccalaureate degree in nursing from a NLN-accredited program and a current RN license. Minimum of two years’ experience, at least one in public health, home care, or hospice nursing, is preferred.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Develops and revises the plan of care with patient and family; initiates plan of care and revisions as status changes.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs; provides health care instructions.
• Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for assigned caseload.
