Job summary

Affinity Hospice Management LLC in Ohio seeks a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will perform comprehensive assessments and develop care plans with family involvement, coordinating with physicians and the interdisciplinary team.

You will provide nursing care, administer treatments, educate families, and ensure safety and quality in patient care throughout the hospice journey.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current RN licensure in the relevant state.
• 1+ year of nursing experience in the last 3 years or a BSN with current RN license.
• Minimum two years' experience preferred, at least one in public health, home care, or hospice.

Responsibilities

• Conduct initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care by applying the nursing process.
• Develop and revise care plans with patient/family involvement; coordinate plan of care.
• Administer medications and treatments per physician's orders
• Educate patients and families on self-care and health maintenance; ensure safe home environment.
