Job summary

The Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home settings across Ohio. The role emphasizes community health education and comprehensive patient/family assessments, with care planned and delivered in partnership with families.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• One year of professional nursing experience in the last three years or a BSN with RN license; two years in public health/home health/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of nursing process.
• Develop and revise the plan of care with patient/family involvement.
• Initiate the plan of care and revise as patient needs change.
• Use health data to determine nursing diagnoses and plan-of-care outcomes.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing needs and self-care; provide health instructions.
• Assist patient with activities of daily living and support comfort care.
• Act as Case Manager when assigned to coordinate care for caseload.
• Complete accurate clinical notes and communicate with physician and care team.
• Attend interdisciplinary group meetings and ensure continuity of care.
