Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, direct and coordinate hospice care in patients' homes across Ohio. The role emphasizes comprehensive assessments, care planning and interprofessional collaboration.

You will administer medications, educate families, participate in on-call coverage, and supervise aides as assigned, while maintaining detailed clinical documentation and ensuring quality end-of-life care.

Qualifications

• Graduate of NLN-accredited school of nursing.
• Current licensure in the state.
• RN with 1+ year of recent professional nursing experience, or BSN with RN licensure; preferred in home health/public health/hospice.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments.
• Plan, implement and revise the plan of care.
• Administer medications and treatments as prescribed.
• Educate patient and family on self-care and care needs.
• Maintain accurate clinical notes and documentation.
• Participate in on-call duties and care coordination.
• Supervise hospice aides as assigned.
