Job summary

Qualifications

Responsibilities

• Completes initial and ongoing comprehensive patient assessments.
• Develops and revises care plans with patient and family.
• Administers medications and treatments per physician orders.
• Coordinates care with the hospice interdisciplinary team.
• Provides patient and family education on self-care and comfort.
• Acts as Case Manager when assigned and ensures continuity of care.
