Job summary

Qualifications

• Graduate of an NLN-accredited school of nursing.
• Current licensure as a Registered Nurse in the state.
• One year of professional nursing experience in the last three years OR a BSN with current RN license; preferred public health/home/hospice experience.
• Supervisory experience not required.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Initiate and revise the plan of care as patient status changes.
• Coordinate patient care as case manager when assigned.
• Communicate with physicians and the interdisciplinary team; educate patients and families.
• Participate in on-call duties and ensure equipment and services are arranged.
• Maintain patient safety and quality of care through ongoing education.
