Job summary

Affinity Hospice Management LLC in Ohio is seeking a dedicated Registered Nurse to plan, organize, and direct hospice care in home and community settings. Qualifications

• Graduate of NLN-accredited nursing program.
• Current state RN licensure.
• 1 year of professional nursing experience within last 3 years; or BSN with current RN license; 2 years preferred in public health/home care/hospice.
• Supervision of hospice aides not required.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Develop and revise plan of care using nursing process.
• Coordinate care with physicians and interdisciplinary team; monitor patient status.
• Administer medications and treatments as prescribed by physician.
• Educate patient and family on care, self-care and comfort options.
• Document clinical notes and report changes to physicians.
• Participate in on-call duties and quality improvement activities.
