Job summary

Affinity Hospice Management LLC in Ohio seeks a registered nurse to plan, coordinate and deliver hospice care in patients' homes. You will perform comprehensive assessments, develop care plans, administer medications, and educate patients and families to support comfort and quality of life.

You will collaborate with an interdisciplinary team, participate in on-call duties, and supervise hospice aides as needed.

Qualifications

Graduate of NLN-accredited nursing program.
Current RN license in Ohio.
Experience in hospice, home health, or public health preferred.

Responsibilities

Conduct initial and ongoing patient/family assessments to determine hospice needs.
Develop and revise plans of care based on assessment and patient needs.
Administer medications and treatments as prescribed by physicians.
Educate patients and families on care, medications, and self-care.
Coordinate with the interdisciplinary team to meet emotional and care needs.
Participate in on-call duties and supervise hospice aides as needed.
