Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities in Parma, OH. The role emphasizes community health education and coordinating care with an interdisciplinary team.

The RN will assess patients, develop and revise care plans, administer medications, counsel families, and participate in on-call duties as defined by policy. A state RN license and NLN-accredited program graduate are required.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current licensure in State.
• 1+ year RN experience in the last 3 years; or BSN with RN license; 2 years in public health/home care/hospice preferred.

Responsibilities

• Complete comprehensive initial and ongoing assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and reevaluate patient and family needs; revise plan of care as necessary.
• Initiate and revise the plan of care as patient status changes.
• Use health assessment data to determine nursing diagnosis.
• Develop goals-based care plan including family in planning.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family on nursing needs and self-care.
• Provide health instructions per plan.
• Assist with activities of daily living and comfort care.
• Act as Case Manager coordinating care for assigned caseload.
• Document accurate clinical notes and pain/symptom management changes.
• Communicate with physician regarding patient needs and orders.
• Coordinate with community health team; educate on self-care techniques.
• Maintain a safe environment and ensure continuity of care.
• Participate in on-call duties and arrange equipment/services.
• Supervise ancillary personnel when required.
• Commit to ongoing professional development.
