Job summary

St. Croix Hospice is seeking an RN Case Manager in Lincoln, NE to lead each patient’s hospice care team and to develop and manage the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RN Case Manager provides day-to-day patient care, administers medications, maintains records, and directs other team members to meet changing needs, focusing on comfort and quality of life.

Qualifications

• Current RN license in state(s) of practice.
• Graduate of a professional school of nursing.
• Ability to pass DHS background, consumer report, and drug screen.
• Valid State Driver’s License and reliable vehicle.

Responsibilities

• Lead the patient’s hospice care team and develop the Plan of Care.
• Provide day-to-day patient care and administer medications.
• Educate patients and families on self-care and the care plan.
• Coordinate care with physicians and interdisciplinary teams.
• Ensure HIPAA compliance and maintain accurate documentation.
