St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

You will provide day-to-day patient care, administer medications, maintain records, and guide other care team members to meet changing patient needs with a focus on comfort and quality of life in a home setting.

Qualifications

• Must have a current RN license in state of practice.
• Graduate of a professional school of nursing and licensed.
• Valid State Driver’s License and access to reliable transportation.
• Ability to pass DHS background check, consumer report, and drug test.

Responsabilités

• Provide day-to-day patient care and manage the Plan of Care with the Hospice Medical Director.
• Administer medications and treatment per physician’s plan of care.
• Educate patients and families on self-care, medications, and diet as appropriate.
• Maintain accurate clinical documentation of pain, symptoms and condition changes.
• Coordinate care with physicians, interdisciplinary teams and community partners.
• Participate in on-call visits, attend meetings, and supervise staff as needed.
