Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and manage the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.

You will provide day-to-day patient care, administer medications, maintain records, educate families, and coordinate with physicians and interdisciplinary teams to ensure comfort and quality of life for patients in home or facility settings.

Qualifications

• Must have a current RN license in state(s) of practice.
• Multistate license acceptable; ensure primary state of residence.
• Graduate of professional nursing school and licensed in practice state.
• Ability to pass DHS background check, consumer report, and drug test.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Lead the hospice care team and plan of care.
• Provide day-to-day patient care and administer medications.
• Educate patients and families on self-care and medications.
• Maintain accurate clinical documentation.
• Coordinate care with physicians and interdisciplinary teams.
• Ensure patient safety and HIPAA compliance.
• Participate in on-call visits and team meetings.
• Supervise ancillary personnel as needed.
