Job summary

Qualifications

• Current RN license in state(s) of practice.
• Ability to pass DHS background check and drug test.
• Driver’s license and reliable automobile required.
• Experience in hospice/home care preferred.

Responsibilities

• Provide day-to-day patient care and med administration as prescribed.
• Develop and revise the Plan of Care with the Hospice Medical Director and family.
• Coordinate care with physicians and interdisciplinary teams.
• Maintain accurate clinical documentation and HIPAA compliance.
• Deliver emotional support to patients and families.
• Arrange for equipment and supplies as needed.
• Participate in on-call visits and interdisciplinary meetings.
• Supervise ancillary personnel and support professional development.
