Job summary

St. Croix Hospice is seeking an experienced RN Case Manager in Minnesota to lead patient care on end-of-life journeys, coordinating with the hospice medical director, patient, family, and facility staff to implement and adjust the Plan of Care.

You will provide day-to-day nursing care, administer medications, maintain accurate records, educate families, and supervise care team members, including on-call duties evenings and weekends.

Qualifications

• Current RN license in the state(s) of practice.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and access to an automobile.
• Multistate license acceptable; primary state residency governs licensure.

Responsibilities

• Provide day-to-day patient care and manage treatment per the Plan of Care.
• Develop, implement, and revise the Plan of Care with the Hospice Medical Director and care team.
• Educate patients and families on self-care, medications, and care plan.
• Coordinate care with physicians, interdisciplinary teams, and community partners.
• Maintain accurate clinical documentation and HIPAA-compliant records.
• Attend on-call visits and participate in quality assessment activities.
