Job summary

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team and oversee the Plan of Care in collaboration with the Hospice Medical Director, patient, and family.

The role focuses on delivering comfort, quality of life and support through comprehensive nursing care, documentation, and coordination with physicians and staff. Willingness to participate in on-call visits is required.

Qualifications

• Must hold an active RN license in practice states.
• Graduate of a professional school of nursing and licensed in practice states.
• Ability to pass DHS background check and drug test.
• Valid State Driver’s License and access to reliable vehicle.

Responsibilities

• Provides day-to-day patient care and manages treatment per physician plan.
• Leads the patient care team to develop and revise the Plan of Care.
• Maintains accurate clinical documentation of pain, symptoms, and condition changes.
• Coordinates care with physicians, interdisciplinary teams, and community partners.
• Ensures HIPAA compliance and patient confidentiality.
• Participates in on-call visits, including evenings and weekends.
