Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to lead each patient’s hospice care team and manage the patient’s Plan of Care in collaboration with the Hospice Medical Director and family.

You will provide day-to-day patient care, administer medications, maintain records, and coordinate care with the care team to enhance comfort and quality of life. The RN is expected to participate in on-call visits, engage in interdisciplinary meetings, and support professional development while

Qualifications

• Must have a current RN license in state(s) of practice.
• Multistate license holders can practice in other states without additional licenses.
• Ability to pass DHS background, consumer report, and drug test.
• Valid State Driver’s License and access to an automobile.

Responsibilities

• Provides day-to-day patient care and manages treatment per the plan of care.
• Administers medications and treatments as prescribed.
• Educates patients and families on self-care.
• Maintains accurate clinical documentation of pain, symptoms, and condition changes.
