Job summary

St. Croix Hospice in Lincoln, Nebraska seeks an experienced RN Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, and family.

The RN Case Manager provides day-to-day patient care, manages treatments, and directs team members to meet changing needs with comfort and dignity. You will ensure accurate documentation, coordinate with physicians and partners, and educate families on medications, care plans,

Qualifications

• Must hold a current RN license in state(s) of practice.
• Multistate license holders may practice in other states where allowed.
• Graduate of a professional nursing program and licensed in practice states.
• Ability to pass DHS background, consumer report, and drug tests.

Responsibilities

• Provide day-to-day patient care and manage treatment per plan of care.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical documentation and patient records.
• Coordinate care with physicians, interdisciplinary teams, and community partners.
• Educate patients and families on self-care, medications, and diet.
