Job summary

St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RNCM provides day-to-day patient care, manages treatments, administers medications, and directs other team members to meet changing patient needs, focusing on comfort and quality of life.

Qualifications

• Current RN license in state(s) of practice.
• Nurses with multistate license may practice in other states while maintaining primary state license.
• Graduate of a professional school of nursing and currently licensed.
• Ability to pass DHS background, consumer report, and drug test.

Responsibilities

• Provide day-to-day patient care and manage treatment.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical documentation and patient records.
• Coordinate care with physicians, interdisciplinary teams, and community health partners.
• Provide emotional support and counseling to patients and families.
• Arrange for necessary medical equipment and supplies.
• Ensure HIPAA compliance and patient confidentiality.
• Participate in on-call visits evenings and weekends.
