Job summary

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family and staff.

The RN Case Manager provides day-to-day patient care, administers medications, maintains records and directs team members to meet changing needs, focusing on comfort and quality of life.

Qualifications

• Must hold a current Registered Nurse (RN) license in state of practice.
• Experience coordinating hospice patient care is preferred.
• Strong documentation and HIPAA compliance skills.

Responsibilities

• Leads the hospice care team and manages each patient’s Plan of Care.
• Administers medications and treatments as prescribed.
• Maintains accurate patient records and coordinates with physicians and staff.
• Educates patients and families on self-care and medications.
• Arranges necessary medical equipment and supplies.
• Participates in on-call visits as required.
